We may treat your provision of an invalid email address or the subsequent malfunction of a previously valid address or cancellation of participation in our online banking product as a withdrawal of your consent to receive electronic statements or documents. Any withdrawal of your consent to receive electronic statements will be effective only after we have a reasonable period of time to process your withdrawal.

How to Update Your Records
It is your responsibility to provide us with true, accurate and complete email address, contact, and other information related to this Disclosure and your account(s), and to maintain and update promptly any changes in this information. What this means for you: When you open an account, we will ask for your name, address, date of birth, and other information that will allow us to identify you. We may also ask to see your driver's license or other identifying documents. As a Community Development Financial Institution (CDFI) we are required by federal law to collect information regarding the gender, race, ethnicity and national origin of individuals that utilize our services. You may select one or more designations for "race". The law provides that a lender may not discriminate on the basis of this information. If you are a permanent resident alien please provide a copy of your resident alien ID Card.

Authorities and Purpose for Collecting Information:
RLCF is collecting the information on this application, including social security numbers and other personal information, to make a character and credit eligibility decision in connection with you or your company's application. RCLF may also use social security numbers for the purpose of collecting and reporting on any delinquent fees or other amounts owed RCLF. Failure to provide this information could affect your ability to obtain a loan. For other individuals signing this application, the submission of the social security number is voluntary and failure to provide your social security number may not affect any right, benefit or privilege to which you are entitled.

Routine Uses:
Some of the information collected may be checked against criminal history indices of the Federal Bureau of Investigation. When the information collected indicates a violation or potential violation of law, whether civil, criminal, or administrative in nature, SBA may refer it to the appropriate agency, whether Federal, State, local, or foreign, charged with responsibility for or otherwise involved in investigation, prosecution, enforcement or prevention of such violations.

Notification of the Right to Receive a Copy of the Appraisal
We may order an appraisal to determine the property's value and charge you for this appraisal. We will promptly give you a copy of any appraisal, even if your loan does not close. You can pay for an additional appraisal for your own use at your own cost.

Notification of the Right to Request Specific Reasons for Credit Denial
If your application for business credit is denied, you have the right to a written statement of the specific reasons for the denial. To obtain the statement, please contact businessloans@rclfms.com or by calling 228-896-3386 within 60 days from the date you are notified of our decision. We will send you a written statement of reasons for the denial within 30 days of receiving your request for the statement.

Acknowledgement
I/We acknowledge that Renaissance Community Loan Fund may verify any information contained in this application or obtain any information or data relating to the Loan, for any legitimate business purpose through any source, including a source named in this application or a consumer reporting agency. Renaissance Community Loan Fund may make inquiries to obtain business reports on the business entity named in this application.

Consent and Release Form, Nonpublic Personal Information Form
I, (Applicant/Owner/Guarantor) do hereby consent to and authorize Renaissance Community Loan Fund (RCLF), its partners, affiliates, agents, contractors and their respective assigns as part of my application for the Program, to request, access, review, disclose, release and share any and all Nonpublic Personal Information ("NPI"), whether provided by me in this application or by additional outside third parties with whom I may or may not have a relationship, as necessary for final determination of my eligibility for an the amount of assistance under the Program. I understand and acknowledge that any party disclosing information to RCLF on my behalf is not responsible for any negligent misrepresentation or omission and I agree to hold any such disclosing party harmless from and against all claims, actions, suits or other proceedings, and any and all losses, judgements, damages, expenses or other costs (including reasonable counsel fees and disbursements), arising from or in any way relating to their disclosure. As part of this consent, I further authorize RCLF, and any other financial institution, lender, insurer, other government agency (federal and state), credit bureau, financial service provider or any other third party to obtain, use and disclose any of my NPI in their possession, to enable RCLF to administer the Program and to enable RCLF to process my application. I understand and acknowledge that RCLF may obtain, use and disclose any NPI received in its investigation of my application with third parties, including those referenced above, as necessary for final determination of my eligibility for and the amount of assistance under the Program.I acknowledge that I have received and reviewed RCLF's privacy policies as they relate to my NOP and my right to privacy associated therewith. I also understand and acknowledge that, as part of those policies, my consent may be revoked at any time with written notice to, as applicable, RCLF. I further understand and acknowledge that any such revocation of this consent may affect my ability to receive assistance under the Program. By completing and signing this application, I acknowledge and agree to the above and agree that this consent may be furnished on my behalf to any financial institution, lender, insurer, government agency (federal or state), credit bureau, financial service provider or other third party.

FLOOD DISASTER PROTECTION ACT OF 1973
I/We hereby acknowledge that we have been advised of the Flood Disaster Protection Act of 1973 and the requirements that I/We provide such insurance coverage on any property located within an area designated as a Flood Hazard Area. Should the subject property fall within a flood hazard area as defined in the Act, then I/We authorize Renaissance Community Loan Fund (RCLF), its successors and/or assigns to purchase such insurance and I/We further agree to pay promptly the cost thereof.

QUALITY CONTROL RELEASE AND AUTHORIZATION TO RE-VERIFY
We, the undersigned borrowers, understand that our application may be selected by the Lender and/ or its assigns or its agents for a quality control review. This review is designed to produce and maintain quality service for our borrowers and to comply with agency and investor guidelines. The quality control review will involve verification of all credit information (including employment history, income, bank accounts, and credit references) as well as the property valuation. We agree to cooperate with the Lender and/ or its assigns or its agents: to the extent necessary to accomplish this review. It is understood that the information may be verified with third parties such as our employers, depository institutions or a credit reporting agency. We therefore have given authorizing the release of employment and/ or financial information to assist in the quality control review process.

PROMOTION, ADVERTISING AND PUBLICITY CONSENT AND RELEASE FORM
I agree and consent that Renaissance Community Loan Fund, and its representatives and assigns, may photograph me/my family/my home/my business during the purchase and loan closing process and use the likeness and/or photography, as applicable, or any reproduction (full or partial) thereof, in any form, style, or color, together with any writing and other advertising, publicity or promotional material, including television, in connection therewith. This consent and release is given without limitation upon, or liability for, any use for advertising, illustration, publication or broadcast of every kind, or in trade, media, digitally or for any purpose. I further agree that such photography and/or likeness shall be and remain the exclusive property of Renaissance Community Loan Fund. I understand that this consent is voluntary and I will not be compensated for the taking of or use of any photograph or reproduction.

Acknowledgement and Certification
The undersigned hereby certifies that the information contained in this application and related materials is true and correct. The undersigned hereby further certifies that the proceeds of any loan made as a result of this application will be used for legal business purposes only and will not be used for personal or consumer purposes. The undersigned hereby affirms that he/she does not discriminate on the basis of race, religion, sex, handicap, sexual preference, or marital status. The undersigned hereby acknowledges that (1) no commercial loan officer has the authority to commit RCLF to any loan without prior approval and (2) any loan commitment must be in writing and signed by an authorized representative of RCLF.

Representations and Warranties
The undersigned hereby authorizes RCLF or any of its affiliates to make inquiries with credit bureaus and others it deems necessary - including landlords, business counselors, consultants and partnering agencies or any other lawful purpose covered under the Fair Credit Reporting Act (FCRA) - to verify the accuracy of the information provided herein and to determine credit worthiness. Further, the undersigned hereby certifies that the enclosed application information is valid, accurate and complete. The undersigned hereby further certifies that the proceeds of any loan made as a result of this application will be used for business purposes only and will not be used to cover any existing personal or consumer debt, general fees related to the preparation of this document or personal or consumer purchases.
